Side-stapled wrappers. No cover attribution. As Clark Coolidge explains in an interview with Tom Orange, the book:
"was a collaboration with me and Larry Fagin. I think Larry started noticing two-word groups in my poems that sounded like names. Some were, but most of them weren't. And he started making up this list. I think 'Lavoris Tonto' was the original sequence, but we changed it to 'Tonto Lavoris.' That was a collaboration, but all the words come from my poems."
